Pelvic floor issues are often associated with postpartum recovery or ageing.
But increasingly, we see a different presentation — particularly in Hong Kong’s active professionals and recreational athletes.
Young men in their 20s and 30s.
Women returning to high-intensity training.
Gym-goers pushing performance goals.
In high-intensity environments, cues like “brace harder” and “stay tight” are common.
But what happens when bracing becomes constant?
The “Always Tight” Pattern
During lifting, sprinting, or intense training sessions, increased abdominal pressure is necessary.
The pelvic floor helps manage that pressure.
However, when:
- Breath-holding becomes habitual
- Core bracing never fully switches off
- Recovery is limited
- Work stress remains high
Baseline muscle tone can gradually increase.
This may contribute to:
- Groin discomfort
- Pelvic heaviness
- Testicular ache in men
- Urinary urgency despite clear tests
- Lower abdominal tightness

Strong Doesn’t Mean Constantly Engaged
The pelvic floor needs to contract — but it also needs to relax.
Without adequate relaxation:
- Circulation may reduce
- Local sensitivity may increase
- Symptoms may appear after activity rather than during
Pain later in the day can be a sign of accumulated load, not weakness.
